Rythmol SR and Breastfeeding

The manufacturer of Rythmol SR (propafenone SR) recommends that women not use this drug while nursing. However, some experts consider normal doses of the drug would likely be safe for use while breastfeeding. Although Rythmol SR has been shown to pass through breast milk, the specific effects that might occur in the nursing infant are unknown at this time.

Can Breastfeeding Women Take Rythmol SR?

Rythmol SR® (propafenone SR) is a prescription antiarrhythmic medication. It is used to maintain a normal heartbeat in people with atrial fibrillation (a type of heart arrhythmia). Rythmol SR passes through breast milk in humans.

Propafenone, the active ingredient in Rythmol SR, has been shown to pass through breast milk in clinical studies. In these studies, small amounts of the drug were found in the breast milk of women who took the medication while lactating. Based on these studies, it has been estimated that a nursing infant would consume between 0.03 percent and 0.1 percent of the mother's Rythmol SR dosage.
Unfortunately, neither study looked at the potential effects on a nursing infant. So, even though it is known that this drug passes through breast milk, it is unknown whether breastfeeding during Rythmol SR treatment would harm a nursing child.
The manufacturer of the medication recommends that women either not take Rythmol SR or not breastfeed. This is because Rythmol SR can cause potentially serious side effects, and the potential for those side effects in a nursing infant cannot be completely ruled out at this time. However, some experts suggest that, at normal doses, the medication would likely be safe for breastfeeding, especially if the nursing child is older than two months of age.
If your healthcare provider recommends using Rythmol SR while breastfeeding, make sure to closely observe your child. Let your child's healthcare provider know if you notice any potential Rythmol SR side effects.
