Metoprolol Dosing

There are several possible dosages of metoprolol; the one your healthcare provider recommends will depend on the medical condition being treated and your age, among other things. For the treatment of high blood pressure with immediate-release metoprolol, dosing recommendations generally start at 100 mg once a day or 50 mg twice a day. With extended-release metoprolol, the recommended starting dosage is 25 mg to 100 mg once daily.

Dosing With Metoprolol: An Introduction

The dose of metoprolol (Lopressor®, Toprol-XL®) your healthcare provider recommends will vary, depending on a number of factors, including:
 
  • Your age
  • The form of metoprolol (immediate-release or extended-release)
  • The medical condition being treated
  • Other medical conditions you may have
  • Other medications you may be currently taking
  • How you respond to metoprolol.
     

Metoprolol Dosage for High Blood Pressure

The recommended starting dose of immediate-release metoprolol tartrate (Lopressor) for people with high blood pressure is 100 mg once a day or 50 mg twice a day. With extended-release metoprolol succinate (Toprol-XL), the recommended starting dosage is 25 mg to 100 mg once a day. After a week or longer, based on the blood pressure response and/or metoprolol side effects, the metoprolol dosage may be increased or decreased. Your healthcare provider may also add a diuretic (a "water pill") or another blood pressure medication if your condition is not controlled by metoprolol alone.
 
The maximum recommended daily dose of immediate-release metoprolol for this use is 450 mg, and the maximum daily dosage of extended-release metoprolol is 400 mg.
 

Metoprolol Dosage for Angina

The recommended starting dose of immediate-release metoprolol for people with angina is 50 mg twice a day. For the extended-release version, the suggested starting dose is 100 mg once a day. After one week, based on the response and/or side effects, the metoprolol dose may be increased or decreased. Your dose may continue to be increased until your angina symptoms improve.
 
The maximum daily dose of both immediate-release and extended-release metoprolol for treating angina symptoms is 400 mg.
