Dofetilide Dosage

The recommended starting dosage of dofetilide for adults with atrial fibrillation or atrial flutter is usually between 125 mcg and 500 mcg twice daily. However, the specific amount will depend on how well your kidneys are functioning and various other factors. Because this medication can cause potentially dangerous reactions, your first few doses will be administered in a hospital where you can be carefully monitored.

An Introduction to Your Dosage of Dofetilide

The dose of dofetilide (Tikosyn®) your healthcare provider recommends will vary, depending on a number of factors, including:
  • Your heart rate
  • Your kidney function
  • Other medical conditions you may have
  • Other medicines you are taking.
As is always the case, do not adjust your dosage unless your healthcare provider specifically tells you to do so.

Recommended Dofetilide Dosing Guidelines

The recommended initial dofetilide dosage for treating atrial fibrillation or atrial flutter is usually between 125 mcg (0.125 mg) and 500 mcg (0.5 mg) twice a day, depending on how well your kidneys are functioning. You will be started on this dose in the hospital.
Two to three hours after you receive your first dose, your healthcare provider will monitor your heart rate and may reduce your dofetilide dosage based on the results of this monitoring. He or she will continue to monitor your heart rate in the hospital for at least three days. Make sure to continue taking the amount your healthcare provider recommends after you leave the hospital.
Your healthcare provider will continue to monitor your kidneys and heart rate at least every three months. If your kidney function worsens, your healthcare provider may recommend you stop taking dofetilide or take a lower dose. He or she may also recommend you stop taking the drug if you develop an abnormal heartbeat.
